This letter is to notify you that the Basic Business License associated with the following rental property is up for renewal in \[**INSERT 90/60**\] days:

In order to renew the Basic Business License**,** you must obtain and pass an inspection prior to renewal. The DOB Inspection and Compliance Administration will contact you directly to schedule an inspection after you submit this online request form[\[WD(3\]](#_msocom_3) , an inspector will contact you [\[WD(4\]](#_msocom_4) [\[WD(5\]](#_msocom_5) to schedule the exact date and time of your inspection.

[\[CT(1\]](#_msoanchor_1)Do we charge the fee if the inspection has to be rescheduled?

[\[WD(2\]](#_msoanchor_2)There is no re-inspection by definition if one did not take place. It would seem to me that this would NOT apply if the person reschedules with sufficient notice, i.e. two or more days notice. Is this correct?

[\[WD(3\]](#_msoanchor_3)Makes it seem like a survey and may be perceived as optional. Also despite the use of “short” questionnaire has a connotation of it being a bit longer.

[\[WD(4\]](#_msoanchor_4)How? The customer needs to know how to expect this contact.

 [\[WD(5\]](#_msoanchor_5)

[\[WD(6\]](#_msoanchor_6)I defer to the group, but this is obsolete since it is being added to the form.

[\[CT(7\]](#_msoanchor_7)Which is it? Revoked or canceled?
